Wrinkle relaxing injections are a type of wrinkle reduction treatment that has revolutionised our approach to facial ageing, particularly in the upper face. This anti-ageing treatment works by injecting tiny amounts of botulinum toxin into specific facial muscles, which causes the muscles to relax. This reduces the appearance of the overlying wrinkles.

Contrary to popular belief, this does not paralyse muscles, but rather, it relaxes them.

Often done in conjunction with dermal fillers (injectable facial fillers), the art of injecting botulinum toxin lies in altering the delicate balance between the muscles of facial expression to reduce lines and wrinkles. These muscles can broadly be divided into two main groups: those that lift the face up and those that pull the face down.

The goal of these injections is to weaken the muscles that pull the face down, thus enhancing the effect of the muscles that lift the face up.

While Botox is generally well-tolerated, some patients may experience mild side effects. Common issues include temporary bruising, slight swelling, or localised discomfort at the injection site. It is quite common for patients to experience a headache following treatment. This is usually very mild and short-lived, and doesn’t normally stop patients coming back for more. Much less commonly, it is possible to develop a droopy eyelid or droopy eyebrow as a side effect of treatment in the upper face, or difficulty with speech and swallowing with treatment in the lower face. Whilst these complications are frustrating, they are thankfully rare and wear off with time. After Botox treatment, you should avoid rubbing the area or wearing a hat that compresses the area immediately after treatment until the drug has been absorbed. You can tell when the drug has been absorbed by watching the little bumps on the skin when it’s first injected disappear. After the first few hours, however, you can resume normal activities. When Botox was first launched, we used to have much more cautious aftercare instructions, like no exercise and no flying for a while, but experience has taught us that this is not necessary. You can get back to most normal activities almost immediately following your treatment.

Treatment of mild frown lines.

Frown Lines

The frown lines are one of the most commonly treated areas using wrinkle relaxing injections.

The muscles that we use to frown also depress the brow. By relaxing them and preventing that effect, we can simultaneously lift the brow, making the eyes look more open and awake

Treatment of forehead lines.

Forehead

The forehead is another area that is commonly treated with wrinkle relaxing injections.

The forehead muscle – the frontalis – is the only muscle we have for lifting the brow. If we relax it completely, it will cause the brow to drop. It is therefor important to only treat a section of the forehead muscle with anti-wrinkle injections, leaving enough muscle movement to support the brow. Achieving this is a delicate balance and is dependant on the skill and experience of your doctor.

In some cases, where there is significant excess skin and heavy, hooded upper eyelids, it is best not to perform anti-wrinkle treatments on the forehead at all.

Crow's Feet

Crow’s feet can be effectively treated with a wrinkle relaxing treatment.

This works especially well for women in their 30s and 40s. When we get into our 50s, however, the muscles around the eyes become very hyper-expressive compounded by loss of volume from and descent of the cheek.

Our preferred way of addressing this issue is to combine wrinkle relaxing injections with some filler in the cheek. This not only improves the reduction of crows feet, but can also help to increase the time between repeat treatments by using a combination approach.

Mouth Corners

The depressor anguli ori (or DAO for short) is a muscle that goes from the corner of the mouth down towards the jaw.

Talking, eating and certain facial expressions cause it to contract, which pulls the corners of the mouth downward. Relaxing it with anti-wrinkle injections has the opposite effect — it elevates the corner of the mouth. This can be quite a difficult effect to achieve and is best suited to younger patients with fine lines.

Lip fillers and fillers for lipstick lines are another way to focus on the mouth area.

Neck

The superficial muscle of the neck, the platysma, usually contracts to pull the neck and lower face downwards. Treating the neck with wrinkle relaxing injections can help to relax these thick bands, as well as relieve the negative pull on the lower face and neck.

This is not an anti-wrinkle treatment that is suitable for everyone. It is best suited to younger patients who have a thin neck.

Other options are CoolSculpting and a non-surgical facelift.

Common Questions

How Often Should I Have Anti-Wrinkle Treatments?

Most people find that the muscle relaxation begins to wear off at around 3 months. However, when the muscle movement begins to return, there is often a lag period of another month or so before the lines and wrinkles begin to re-appear. It is when the lines and wrinkles begin to re-appear that you should think about having further anti-wrinkle injections.

In the first year of your treatment plan, you will most likely find that you need to have the treatment done approximately 3 times (i.e. not longer than 4 months per treatment). In the long term, however, most patients find they only need to do it twice per year to maintain the results.

Can they be used with other Fillers?

It is very common for us to combine treatment with muscle relaxing injections and other types of non-surgical treatment, such as hyaluronic acid dermal fillers. Occasionally, we will also add in some laser treatments too. Often the key to achieving the best outcome is to employ a combination treatment approach, rather than relying on one individual type of treatment too heavily.

What Are The Side Effects & Complications?

Anti-wrinkle injections as part of a treatment for the upper face are usually an almost pain-free procedure. The needles used are very small and, with good technique, it is normally possible to avoid causing bruising.

One of the most common side effects of anti-wrinkle filler injections is headaches. These can affect up to 1 in 10 people. For most people, it settles down within a day or two, however, it can take up to a couple of weeks for some people. We have seen one or two patients who have found the headaches lasted the whole three-month duration of the treatment. For most people, however, the headaches are very mild and self-limiting, so it is unusual to require any type of painkiller for them.

Complications from anti-wrinkle treatment can occur from the botulinum toxin spreading to nearby muscles that were not intended to be targeted by the treatment. In the upper face, this can cause temporary drooping of the eyelid or eyebrow. Because this usually happens as a result of a small amount of the botulinum toxin from the anti-wrinkle injections spreading to the wrong area, rather than a direct injection into the wrong area, it tends to wear off quite quickly — usually within a few of weeks. In the lower face and neck, it is possible for the toxin to diffuse into muscles which help to move the mouth, resulting in a lopsided smile. Rarely, this treatment in the neck can affect speech and ability to swallow.
