How Does It Work?
What Is a Hairline Transplant?
A hairline transplant is a focused procedure aimed at restoring or enhancing the hairline. It is often requested by younger patients who are at the earlier stages of hair loss, although it is equally an important part of a more comprehensive hair transplant. The hairline transplant is the most aesthetically critical part of any transplant since it is the most visible part of the procedure. It requires special planning and consideration, because we generally want to make it as thick as possible – but the desire to make it thick needs to be balanced with the need to conserve grafts to either cover other areas of hair loss or to meet future transplant needs in younger patients.
Hairline It is a subset of Follicular Unit Extraction (FUE), where individual hair follicles are harvested from the donor area and meticulously placed along the frontal scalp to create a natural-looking hairline. This targeted approach is ideal for patients experiencing receding hairlines or thinning at the temples.
The Hairline Design Process
Designing a natural-looking hairline is both an art and a science. At McKeown Medical, Dr. Mani takes a personalised approach to ensure the hairline complements your facial features and looks entirely natural. Often younger men want a hairline that emulates their teenage hairline: low and straight. However, you need to remember that the hairline that is transplanted now is the hairline you will likely have for the rest of your life. Whilst a low and flat hairline make look good in your 20s and 30s, it will look increasingly unnatural as you move into your 40s, 50s and 60s.
This is a critical design factor that we need to get right from the outset. We want this hairline to look great now – but we want it to look great in the future too. As well as the height and positioning of the hairline, lots of subtle irregularities are key to making it look natural and not ‘stuck-on’.
Dense packing the hairline
Achieving good density at the hairline is critical to making it look natural. The density can be anywhere from 35 – 50 grafts per square centimetre. The decision on density, and the suitability for dense packing, will come down to a number of factors including the age of the patient, family history of hair loss, the likelihood of extensive future hair loss, the availability and quality of the donor site. What we never want to do is use up all of your donor site providing a great hairline, but you have no reserves left to deal with future hair loss.

The Procedure: Step by Step
Initial contact
When you first get in touch with us you will be allocated your own Patient Relationship Manager, who will have extra training in hair transplants and will be able to answer most of your initial questions. The PRM will ask you to send in photographs of the front and back of your head which she will share with Dr Mani and give you an initial idea of the number of grafts required and likely costs. She will answer any questions you have at this stage.
Consultation
If you decide you want to go ahead and have a consultation with Dr Mani, this can be done in-person or via zoom. He will evaluate your hair loss, talk to you about your family history and guide you on what he thinks would be the best possible treatment strategy for you going forward. He will also explain to you the likely results that you can achieve from surgery and will talk to you directly about what can go wrong. This is very important so you can make a fully informed decision to proceed.
Hairline Mapping
If you make the decision to go ahead with surgery, your procedure will begin by mapping out the new hairline. Dr Mani will do this by drawing on your scalp, being careful to incorporate asymmetries and irregularities to make it look natural. This is a two way process, with feedback from you on what you want and would prefer. We will keep refining the design until both you and the doctor are satisfied that the design is right for your face.
Preparation
Next, the donor area is trimmed, and local anaesthesia is injected. The injection of local anaesthetic should be the only part of the procedure where you feel discomfort. If you are particularly sensitive, we can provide some Penthrox, which is a more modern version of “gas and air” to get you through the injections.
Follicular Unit Extraction
Individual follicles are harvested from the donor site, typically the back or sides of the scalp. Dr. Mani personally performs this step to maximise the quality of grafts. In many clinics, some or all of this part of the procedure is delegated to technicians. This is a critical part of the procedure however and Dr Mani performs this himself to ensure the best quality grafts and optimal chances of graft survival.
Recipient Site Creation
Tiny incisions are made along the hairline to accommodate the grafts. Dr Mani does this using sapphire blades, which are thinner and more sharp than traditional steel blades. Although this is a much more expensive manoeuvre, it gives us greater control of the future hair follicles’ direction of growth and allows us to more accurately and safely dense pack the follicles, where appropriate.
Graft Placement
Each graft is meticulously placed following the pre-designed hairline pattern. Dr. Mani personally plants the first 10% of the grafts to ensure the scalp is responding as expected and to evaluate any adjustments that may be required, before allowing the technicians to finish the placement. This ensures natural angles and density.
Post-Procedure Care
After the procedure, we will make sure you get something to eat and drink before you go home. Our clinic nurse will call you the day after your procedure to make sure you are doing OK and we will get you back in the clinic a week later for a wound check to make sure everything looks fine and there are no signs of complications like infection. After that, it’s a matter of waiting for the new follicles to grow. Dr Mani will follow up with you six and twelve months after your procedure to make sure you have achieved the results you were hoping for.

Recovery & Results
The recovery process for a hairline transplant is straightforward. Here’s what to expect:
Initial Healing: Swelling and redness around the transplant site subside within a week. You can resume most daily activities after 48 hours.
Shedding Phase: The transplanted hairs may shed within the first month, a normal part of the growth cycle.
Growth Phase: New hair begins to grow around the three-month mark, with noticeable improvements by six months. Full results are typically visible within 12-18 months.
Common Questions
Will the results look natural?
Achieving a natural looking result is one of the most important aspects of hair transplant in our practice. Our goal is to provide you with realistic looking density and an appropriately designed hairline that complements the rest of your face. Just take a look at the before and after examples of patients treated by Dr Mani.
Is the procedure painful?
The only part of the procedure that is painful are the initial injections of local anaesthetic. We can help with this by providing you with some Penthrox, which is a more modern version of ‘gas and air’. After the injections, you should not feel any more pain. Following the procedure you can experience some mild to moderate discomfort, although this can typically be managed by over the counter pain medications like paracetamol and ibuprofen.
How many grafts are needed for a hairline transplant?
The number of grafts varies depending on the extent of hairline recession. A typical hairline transplant requires 1,200 to 2,500 grafts.
Are there risks involved?
Hairline transplants are generally safe. Potential risks include infection, scarring, or uneven growth, which are minimised by our meticulous techniques and aftercare. There is a small risk of hair transplant failure, although Dr Mani’s personal failure rate is less than 1% which is why he has become the go-to global destination for hair transplant procedures
