Diagnostic hysteroscopy is a minimally invasive procedure used to examine the inside of the uterus (endometrial cavity) for diagnostic purposes. This procedure allows us to identify and evaluate abnormalities that may cause symptoms such as abnormal uterine bleeding, infertility, or recurrent miscarriage.

When Is Diagnostic Hysteroscopy Recommended?

A diagnostic hysteroscopy – essentially going in to take a look around – can be helpful in the following circumstances:

  • Abnormal Uterine Bleeding: Heavy, prolonged, or irregular periods or bleeding between periods.
  • Infertility: Difficulty conceiving without an apparent cause.
  • Recurrent Miscarriages: Two or more consecutive pregnancy losses.
  • Suspected Uterine Abnormalities: Conditions such as fibroids, polyps, adhesions, or septa that may impact uterine health or fertility.
  • Postmenopausal Bleeding: Any vaginal bleeding after menopause that requires evaluation.
  • Abnormal Imaging Results: Follow-up for findings from ultrasound or other imaging studies suggesting uterine irregularities.

What Happens During a Diagnostic Hysteroscopy?

Diagnostic hysteroscopy is typically performed on an outpatient basis and does not require incisions. Here is what you can expect:

Preparation
We perform the procedure under local anaesthetic as an outpatient. We are the first private service in Scotland to offer this type of service. This is the normal standard in most of America because it is less invasive, more convenient and less expensive for patients, compared with being admitted to hospital for general anaesthetic.

Local anaesthetic is injected around the cervix, which should be the only discomfort you experience. We also offer IV sedation performed by a consultant anaesthetist if you would prefer to be sleepy throughout, although if you choose this option you will need a responsible adult to take you home and stay with you overnight. We will normally try to schedule the procedure shortly after your menstrual period to provide the clearest view of the uterine cavity.

During the procedure
A hysteroscope – a thin, lighted tube – is gently inserted through the vagina and cervix into the uterus. Saline or carbon dioxide gas is used to expand the uterine cavity, providing a clear view of the uterine lining and structures. Dr Iain then visually examines the uterus for abnormalities and may take biopsies or images if needed.

Duration
The procedure typically takes 10 to 30 minutes, depending on its complexity and whether additional steps, such as biopsies, are performed. If a polyp or fibroid is discovered, it may be possible to convert to a MyoSure® procedure and have it treated at the same session. This will be discussed with you before your procedure.

Recovery
Patients go home the same day, typically half an hour after their procedure. Mild cramping or spotting may occur for a day or two. Normal activities can usually be resumed within 24 to 48 hours.

What to Expect

What will happen at my initial consultation?

Your consultation will begin with a detailed medical history so Dr Iain can understand:

  • What your symptoms and how they impact your daily life.
  • Your medical and surgical history, especially any gynaecological issues or procedures.
  • Any plans for pregnancy or fertility preservation.

After finding out about your medical history, Dr Iain will then go on and perform an ultrasound scan of your uterus and ovaries to look for any abnormalities that are visible on scan before making a recommendation for next steps.

Possible complications of diagnostic hysteroscopy

While diagnostic hysteroscopy is generally safe, there are some potential risks, including:

  • Infection: A slight risk of uterine or pelvic infection.
  • Bleeding: Light bleeding is normal, but excessive bleeding is rare.
  • Uterine Perforation: Rarely, the hysteroscope may create a small tear in the uterine wall.
  • Reaction to anaesthesia: Depending on the type of anaesthesia used, minor side effects or allergic reactions may occur.
  • Cramping and Discomfort: Mild cramping during or after the procedure is common.

You should always contact us after your procedure if you experience fever, severe pain, heavy bleeding, or other unusual symptoms after the procedure.
